Starting January 1, 2026, Minnesota Paid Leave will offer payments and job protections to people who need time away from work for their own health or to care for a family member. Paid Leave helps Minnesotans stay financially stable while caring for their own well-being or being there for the people they love.

When you apply for Paid Leave, you will need to provide a certification document. For most types of leave, this will be a certification form. These forms verify your reason for taking leave.

Each type of leave has its own form. You only need to complete the form that matches the type of leave you are requesting. Your healthcare or service provider will also need to fill out part of the form and sign it.
